The asthma action plan chosen should be appropriate for the person’s age, treatment regimen, asthma severity, culture, language, literacy level, and ability to self-manage.
Our template is below, along with links to alternative options. Please contact us if you would like your organisation’s asthma action plan considered for inclusion in this section.
It is Department of Education and Training’s policy that schools and childcare centres in Australia should accept any valid asthma action plans, if the plan meets the standard set in The Australian Asthma Handbook.

We are working towards inclusion of the asthma action plan in medical prescribing software programs. In the meantime, you may wish to save an electronic version of the action plan in your practice's prescribing software. The PDF version can be saved as a patient education resource. The RTF version can be saved as a letter writer template, which may then need amending to suit your practice’s administrative processes.

This asthma action plan is specifically for patients using the combination medication Fostair® (beclometasone and formoterol) as a maintenance and reliever therapy.
Note: Fostair® is approved for use by adults 18 years and older in Australia

This asthma action plan is specifically for patients using the combination medication Symbicort (budesonide and eformoterol) as an anti-inflammatory reliever with or without maintenance.
Note: Symbicort is not recommended for use in Australia in children under 12 years.
